Life insurance is not something anyone wants to ever have to use. Berger Health provides a comprehensive life insurance benefit at no expense to you along with options for you to purchase additional amounts at a group discounted rate.
We are pleased to provide comprehensive life insurance benefits – an extremely important benefit – at no cost to you. Employees receive an amount equal to a multiple of their annual earnings, depending on the classification and job title that you maintain.
Employee Paid Optional/Supplemental Coverage
In addition to the paid life and accidental death and dismemberment benefits, you may qualify to purchase optional coverage amounts up to five times your annual earnings or $500,000 – whichever is less. Initial enrollment allows for up to $180,000 on a guarantee issue basis. Amounts in excess of $180,000 will require that you satisfy Evidence of Insurability. If you choose to enroll in optional life coverage as an employee, you may also elect to insure your spouse and dependent children.
Group Life/Accidental Death & Dismemberment
Berger Health System provides eligible employees with a company-paid Life and AD&D benefit. In the event of an employee’s death, listed beneficiaries will receive a benefit in an amount based on an employee’s salary and employment classification.
Voluntary Life/Accidental Death & Dismemberment
Currently Enrolled Members are eligible for the opportunity to elect additional Term Life/AD&D at their own expense (through payroll deduction) for themselves and their spouse. Enrolled employees may increase their existing benefit by one or two increments of $10,000 up to (the lesser of) 5x annual compensation or $180,000 without Evidence of Insurability. Spouses can increase by one or two units of $5,000 up to $30,000 without Evidence of Insurability.
